Meeting Verification

Documenting & Submitting Meeting Verification:

All meeting verifications are submitted via email to: mymeetings@claritymhr.com.

In the subject line include:

  • Total meetings for the month to date / Grand Total to Date
  • Example Subject Line: June: 3 / Total 45

**Do not send AA verifications to your counselor’s email

In Person Meetings:

  • Meeting attendance is recorded on a signed attendance sheet or a meeting slip, which include the date, time, location, and signature of the meeting facilitator or a group member authorized to verify attendance.
  • Meeting slips are submitted via email at: mymeetings@claritymhr.com
    Upload meeting slip and attach to email OR
    Take a clear photo of meeting slip and attach to email

Online Meetings:

  • Meeting attendance is verified via email sent by the meeting facilitator to your email address
  • Forward email verification to: mymeetings@claritymhr.com

Meeting Verification Requirements:

    The requirements for documenting court-ordered self-help or mutual support meetings (such as AA/NA) include the following elements:

    1. Verification of Attendance: Individuals are required to provide proof that they attended each meeting (online or in-person).
    2. Accurate Record Keeping: Documentation should clearly list each meeting attended, including the date, time, and location. If a meeting slip or attendance sheet is used, it should be filled out at each meeting and kept for submission to the court, probation officer, or treatment provider.
    3. Submitting Documented Attendance: Regularly providing documented meeting verification to counselor or case manager.

    Content of Documentation:

    At a minimum, documentation should include:

    • Name of the participant
    • Date and time of each meeting attended
    • Name or signature of the verifying individual
    • Name or type of meeting/group

    Keep Your Records!

    Individuals should retain copies of all documentation in case questions arise or additional verification is needed by the court or supervising authority.
