To Schedule a Legally Required SUD Assessment at Clarity Mental Health & Recovery

If you need a legally required Substance Use Disorder (SUD) assessment, Clarity Mental Health & Recovery can help. Here’s what you need to know to schedule your appointment and gather the necessary documents.

What to Expect at Your Assessment Appointment

Assessment Appointment (90-120 minutes)
For your first appointment, a Substance Use Disorder Professional (SUDP) will conduct a detailed assessment based on the ASAM diagnostic criteria to understand your specific needs. As a part of our evaluation, you will also be required to perform a urine analysis to allow us to gather additional insights into any recent substance use.

Your Assessment Summary & Recommendations will be completed once the counselor has received and reviewed your urinalysis results and any other collateral information required. ** Urinalysis lab results can take up to 7-10 business days to be processed.

Follow Up Appointment (45-60 minutes)
The follow up appointment occurs approximately two weeks after your initial assessment. At this appointment the counselor will discuss the results of your assessment and any treatment indicated. The counselor will answer any questions you may have, and provide information about potential next steps.

Steps:

  • Contact Clarity Mental Health & Recovery by phone, email or text and inform the office you need a court mandated SUD evaluation. There will be a $50 fee due at this time as a document fee.
  • New client paperwork will be sent to your email, in this packet you will find an ROI (release of information), please complete this with your attorney’s name and fax number so we can request and gather the necessary documents for your evaluation which includes the arrest report and defendant criminal history.
  • Once your paperwork has been received from your attorney, you will be called to get on the calendar with one of our SUD evaluators.
  • At the time of your evaluation you will also need to bring in a Washington State Driving Abstract.

  • If your attorney does not have this document we will notify you to obtain it
  • A DCH or defendant criminal history can be found at the county clerks office (sometimes a fee may apply)
